A Birmingham nightclub, Oceana had to cough up £85,000 as penalty and £33,000 as legal costs following investigations into the death of a customer outside the club in February 2007.

It was found that the door staff was not adequately trained and there was no safety procedure in place to ensure proper ejection from the club. The club had clearly failed to consider the risk factor associated with the fire exit.
